In career mode, players must respond to emergencies as quickly as possible in order to avoid losing patients. Once an emergency is reported, players must go to the location quickly. Carefully planning which route to take is also important. The player must avoid pedestrians and cars on the way to the scene of the accident, too.

Ambulances used in the game are based on real vehicles. They also contain equipment and medical tools for saving lives. While driving, players must follow the green arrow on screen to get to the location of the accident. There is also a speedometer on screen to monitor the driving speed. Some patients must be taken to the hospital, while other patients require immediate attention on the scene of the accident. Some of the tasks that must be done include stopping the bleeding, clearing the respiratory tract, checking vital signs, administering an IV drip, and many others. Doing the correct medical procedure will give the player points.

Players get the chance to unlock and drive more ambulances as the game progresses.
